<br /> <br />E-RATE LETTER OF AGENCY <br /> <br /> <br />Washington State Library Technology Consortium (LibTech) <br />6880 Capitol Blvd SE <br />Tumwater, WA 98501 <br /> <br />RE: Cybersecurity Amendment to the Letter of Agency for E-Rate Funding Years 2024, 2025, 2026, 2027 & 2028 <br />(July 2024 – June 2029) Rev 2 <br /> <br /> <br />This first amendment to the existing E-rate Letter of Agency confirms our library’s participation with the LibTech <br />Consortium in the new FCC E-Rate Cybersecurity Pilot Program for the procurement of eligible cybersecurity <br />equipment and services for E-rate funding years 2025, 2026 and 2027. This amendment will remain in effect if the <br />Cybersecurity Pilot Program is extended or integrated into regular E-rate programming in the 2028 funding year. <br /> <br />I hereby authorize the Washington State Library and LibTech to submit and to respond to related inquiries on FCC <br />Forms 484 (Part 1 & 2), 470, 471,472/474, 486, 498, 500, SPIN Change and Service Substitution Requests, plus any <br />other related program forms or requirements, from to the Universal Service Administrative Company (USAC) on <br />behalf of my library. <br /> <br />I understand that the Pilot Program rules do not allow applicants to apply as both individual entities and as part of <br />a consortium, so this would preclude my library from applying for individual E-rate Cybersecurity funding.
