Your office has been nothing but a pleasure to work with,often people have a preconceived notion about working with government agencies, my experience here has been the exact opposite of this fear people may have. I took out this permit and September 13, 2018 I received a letter informing me that the permit would expire10.16.18. I requested an extension from your office due to health reasons and one was granted. Due to circumstances beyond my control (we needed special materials, bad weather) health reasons (congestive heart failure) I was not able to complete the project on time. I mistakenly though the permit was extended until 4.16.19. I called your office 3.27.19 and was told that the permit was only good until 3.24.19, I did not receive any mailing warning me of this pending expiration as I did in you letter of 3.13.18. Please do not think I am placing any blame on your office I accept my mistakes. When I called your office I was treated with respect and courtesy,the young lady told me to request an extension. I would formally like to request such an extension at this time. We now have almost all the material (there should be not more problems here),the project is under way and with no more freezing cold and snow as we just experience in the last couple of months God willing this will be done and take care of shortly.
