Scope of Work <br />continued <br />City of Everett 8 <br />Dorn Ave. Drainage Improvements Otak <br />Otak will maintain regular email contact with the utility companies to make them aware of the <br />project and the relocation efforts that may be required. It has been Otak’s experience that utility <br />companies are much more likely to complete their relocations in a timely manner if they feel that <br />they have been included in the process from the beginning. Communication will begin with as- <br />built requests and Otak will send the 30%, 60% and 90% design drawings. It is anticipated that up <br />to six (6) utility coordination meetings will occur with utility companies to review 60% design, <br />associated cost and schedule implications, and design (by others) of relocation (if needed) of the <br />affected utilities. <br />4. Formal Communication Records. <br />Otak will maintain a formal record of all communications with the utility companies associated <br />with each potential conflict or relocation effort. Progress on utility coordination will be tracked in a <br />spreadsheet. <br />5. Utility Coordination Plan <br />Prepare and maintain a Utility Coordination Plan to show all existing and proposed utilities within <br />the project corridor. Otak will also prepare a spreadsheet/matrix summarizing utility conflict <br />locations and actions for each of the project locations. These documents will be updated <br />whenever new information becomes available to assure that all utility coordination activities are <br />tracked. <br />6.2 Utility Potholing Coordination <br />Otak will prepare a pothole plan for coordination and approval by the City and franchise utilities and will <br />contract with a utility locating company for potholing utilities that may be in conflict and have elevations <br />that cannot be determined from available information. It is assumed that up to thirty-eight (38) potholes <br />will be required. Surveyors will survey the nails and hubs (separate task) set by the potholing company <br />for locating the utility for project design. Critical utility crossings are gravity side sewer connections, water <br />lines, and telecommunication duct banks (as needed). <br /> <br />6.3 Utility Potholing (Vendor – APS/BRAVO) <br />Otak will contract with a vendor (APS/BRAVO) for performing potholing field work. Vendor will prepare <br />the temporary traffic control plan and obtain the Right-of-Way traffic control permit for potholing activities <br />and provide temporary traffic control and flagging. It is assumed that up to thirty-eight (38) potholes will <br />be required. <br /> <br />TASK 6 DELIVERABLES <br /> Utility coordination tracking spreadsheet. <br /> Electronic spreadsheet/matrix summarizing utility conflict locations and actions for each of the <br />locations. <br /> Utility Coordination Plan graphic (electronic pdf) including pothole information. <br /> <br />TASK 6 ASSUMPTIONS <br /> Meetings with power, gas and telecommunications will be separate meetings (2 each – total 6).
