CHANGE ORDER/AMENDMENT # Contract (Procurement) Number: Services Agreement Contract Title: RFP #2024-020 Bus Schedules Printing Description of Change: As allowed by Section 2.4 Changes in Scope of Work of the RFP: 1. Added: the finished booklet size of 80 inside pages plus a 6 -page cover. Pricing for the following print quantities shall be as follows: a. 15,000 booklets for $9,709.87. b. 20,000 booklets for $11,434.81. 2. Base year pricing shall remain unchanged, as shown in the original proposal submitted. Regardless of the date(s) on which this Change Order/Amendment is signed by the parties, and regardless of any Agreement completion date(s) that may have been in the Agreement prior to this Amendment, the parties agree that the Agreement is deemed continuously in effect since the Original Agreement Date. All provisions in the Agreement shall remain in effect except as expressly modified by this Change Order/Amendment. From and after the effective date of this Change Order/Amendment, all references to the Agreement in the Agreement are deemed references to the Agreement as modified by this Amendment. By signing below, City of Everett and the Service Provider agree that the Agreement shall be changed to the extent described in the above “Description of Change.” This Change Order/Amendment only changes the Agreement between City of Everett and the Service Provider to the extent explicitly provided in this Change Order/Amendment.
