It appears the monument lid had been removed from traffic activity which I <br /> assume may be snowplow activity over the winter. This previous permit is what we used to repair a <br /> similar location in the ROW a bit further to the east on 41st Street. This location is closer to the cross <br /> walk at the southwest corner of the intersection of 415t street and Rucker Ave. I consider this to be <br /> a safety issue that should be addressed as soon as possible to prevent damage or injury pedestrian <br /> and vehicle traffic. I am hoping to get this scheduled as soon as Friday March 17. <br /> Please renew the previous permit and let me know the fees needed to repair this issue.
