Job Steps Hazards Controls C <br /> Cut and cap Release of fuel oil. Ensure fuel pipe is properly drained prior to disconnection. Mobilize spill kit in immediate vicinity. Place <br /> piping, if containment beneath operations. <br /> discovered. L <br /> Injury from hand or Hand and power tools shall be used, inspected, and maintained in accordance with the manufacturer's instructions <br /> power tools. and recommendations and shall be used only for the purpose for which designed. Hand and power tools shall be in <br /> good repair and with all required safety devices installed and properly adjusted. <br /> Inert tank. Explosion and/or fire. All inerting activities to be performed by a Certified Marine Chemist. All potential sources of ignition will be <br /> eliminated from the area prior to starting inerting activities: smoking will be banned in the area, all open flame and <br /> spark-producing equipment at least 50' in all directions of tanks will be shut down, and all electrical and internal <br /> combustion equipment will be removed unless it is intrinsically safe. <br /> Exhaust fumes from inerting procedures will be vented a minimum of 12' above grade and 3' above any adjacent M <br /> roof lines.The atmosphere will be tested at ground level to ensure that vapors are not collecting at ground level. <br /> Following inerting activity, the tanks will be tested with a calibrated four-gas meter to ensure that tank has been <br /> successfully inerted. Oxygen must be reduced below levels necessary to support combustion— at least 12%— 14% <br /> oxygen by volume or an oxygen meter reading of 1%— 10%. Readings will be collected at bottom, middle, and top <br /> of the tank as well as the ends. <br /> Triple-rinse Use of vacuum truck All vacuum truck operators shall be trained in operation of truck and associated equipment. Inspect the truck, <br /> and clean could result in injury. equipment, and loading/off-loading sites prior to beginning the work to assure safe operations. Monitor tank-level <br /> UST. indicators to avoid overfilling and over-pressurizing receiving tanks or creating excessive vacuum in supply tanks. <br /> Ensure the operators and other personnel in the area are adequately protected from the noise through the use of <br /> hearing protection. Prior to starting transfer operations, vacuum trucks need to be grounded directly to the earth <br /> or bonded to another object. <br /> All pumping system operations need to be bonded so that there is a continuous conductive path from the tanks <br /> through the hose and nozzle to the filtration equipment and grounded to dissipate stray currents to earth (ground). M <br /> Prior to starting transfer operations, all pumping equipment must be grounded directly to the earth or bonded to <br /> another object that is inherently grounded (due to proper contact with the earth) such as a large storage tank or <br /> underground piping. <br /> Trips and falls over <br /> hoses and/or risk of Run vacuum hoses out of walking and working areas. If hoses must be run in a walkway,then they must be secured <br /> jumping hoses. so that they can't move unexpectedly. Use mechanical means to handle hoses and equipment whenever possible. <br /> Get help to move hoses and equipment. <br />
