<br />Exhibit B <br />HCS-24-66-2440-198 <br />City of Everett <br />Page 6 of 7 <br />expenditure, or performance issues related to utilization, expenditures, outcomes, <br />or required reports, the County will implement the following corrective action: <br /> <br />A. The County will provide written notice of issues identified and steps that the <br />County will be taking to address those issues, requirements for agency <br />response, timeframe for taking action, and/or identification of other necessary <br />information to address the issues. Subsequent written document may be <br />provide depending on status of the issues and progress toward resolution. <br /> <br />B. A written Corrective Action Plan (CAP) will be developed detailing actions to be <br />completed by the Agency. A written CAP may be a component of the monitoring <br />letter or a separate written document to address any identified issues. The <br />Agency shall provide response to the request for a CAP by the date provided <br />documenting actions in adequate detail for the County to assess whether <br />issues are in the process of being or have been resolved. The County will <br />assess contract and performance issues using all necessary information to <br />determine if the CAP requirements have been met and issues resolved or if <br />further steps are needed. <br /> <br />1. Low-Level Intervention which may include more training or technical <br />assistance, monitoring, more frequent reviews of reports or invoices or other <br />activities to address the concerns. <br /> <br />2. Moderate-Level Interventions will address more serious concerns and may <br />include restricting payments, disallowing costs, repayments, or other <br />actions to address concerns. <br /> <br />3. High-Level Interventions will address the most serious concerns and may <br />include contract suspension or termination, legal action, and could result in <br />not being eligible to apply for funding or receive future funding or other <br />actions that address serious concerns. <br /> <br />C. ADDITIONAL REQUIREMENTS <br /> <br />The Agency shall participate in the following activities: <br />A. Everett/Snohomish County Continuum of Care (CoC) activities, including <br />participation in planning activities, and the annual Point-In-Time (PIT) Count of <br />individuals and families experiencing homelessness or who are at risk of <br />homelessness. PIT participation includes the active recruitment of volunteers <br />to conduct the PIT County and submission of PIT project-specific data and <br />information. <br />B. The Agency shall ensure that appropriate staff attend all applicable trainings <br />and learning opportunities, as identified by the County.
