SFA Design Group, LLC <br />STRUCTURAL I GEOTECHNICAL I SPECIAL INSPECTIONS <br />PORTLAND, OR I LIVERMORE, CA SEATTLE, WA <br />503.641 .831 1 1 www.sfadg.com <br />Special Inspection <br />FINAL SUMMARY REPORT <br />June 2, 2024 <br />ATTN: Inspections and Permitting <br />RE: Project Name: Bankston Residence <br />SFA Project #: MFR23-336 <br />Project Address: 2516 Hillside Ln, Everett, WA 98203 <br />Dear Building Inspection Division: <br />This letter serves to certify that in accordance with Chapter 17 of State and International Building Code <br />requirements outlined in the General Structural Notes on S 1.1 of the approved permit drawings, SEA Design <br />Group, LLC has performed special inspection as detailed below. <br />The inspector verified the installation of (7) Push Piers, (23) pin piles, & (18) Floor Stabilizers. Observations <br />include Push Pier, Pin Pile, & Floor Stabilizer specifications, locations and spacing. All products were installed <br />per the intent of the design and any installation deviations were reviewed and approved by this office with no <br />outstanding non-compliance items. <br />Our inspection and testing services do not constitute a warranty or guarantee of any type and were provided and <br />intended to help reduce the risk of construction defects, deficiencies or omissions that may arise during and after <br />construction. It is the contractor's responsibility to perform their work in accordance with the approved <br />construction documents.
