CHANGE ORDER # <br />Project (Procurement) Number: Small Works Roster Project Title: <br />IFQ 2024-113 Clark Park Renovation and Off-Leash Dog Area <br />Description of Change: (Attach contractor change proposal and cost schedule) (Attach Price or Cost Analysis) <br />BUILDING/LOCATION: 2400 Lombard Avenue, Everett, WA 98201 <br />This change order increases the total project price by including special personal protective equipment (PPE) <br />required to dismantle and wrap lead-effected materials under the soffits of the gazebo. <br />The City conducted a lead paint test to ensure the gazebo was free of lead paint before the contractor <br />disassembled it.
