Suppliers may emphasize in their narrative any areas of their proposal that they believe exceed <br />our requirements. <br />1. Qualifications and Relevant Experience <br />A. Briefly describe your company. Include how long the company has been in business. <br />B. If awarded this contract, who are you proposing will be the primary nutritionist(s) for the Police <br />Skills Refresher training and individual appointments? Include their education, years in the <br />industry, years with the company, years of public speaking, details of their experience, etc. <br />C. Describe your experience working with shift workers, such as nurses, first responders, police <br />officers, or veterans. <br />D. Describe your nutritionist(s)'s proficiency with walking clients through successful behavioral <br />change, such as providing training certifications, testimonials, or other demonstrations of your <br />training and experience. <br />E. Describe your philosophy of practice and what characteristics most distinguish your company <br />from your competitors. <br />2. Technical Capability, Approach, and Capacity <br />A. How will you manage the one-on-one appointment workflow? Describe how employees will <br />schedule appointments with you, what appointment reminders are, the flow of appointments, <br />and how you will collect employee feedback. <br />B. What is your availability for this project? Will the proposed primary nutritionist(s) be <br />available at the times specified in Section 2.3? Explain how you will ensure a nutritionist will be <br />available for individual sessions. <br />C. Provide a sample of a nutritional training presentation that was presented in a group setting. <br />Samples can be delivered via video recording (preferred), copy of slides and handouts, or <br />presentation outline. <br />D. Describe the information you would include in the training presentation. <br />E. Describe how you would conduct individual appointments with officers and civilian staff. What <br />topics are you prepared to discuss other than those listed in Section 2.4? <br />F. How does your approach meet or exceed our needs as described in the scope of work? <br />G. Describe how you plan to reach out to officers and civilian staff to engage them in utilizing <br />nutritional services. <br />H. Describe your approach to recommending or advising on the use of nutritional supplements. Does <br />your company sell supplements directly to clients? If so, <br />percentage off these supplement sales? <br />I. Describe any additional services that your company offers, including training.
