CHANGE ORDER #
Project (Procurement) Number: Small Works Roster Project Title:
IFQ 2024-113 Clark Park Renovation and Off-Leash Dog Area
Description of Change: (Attach contractor change proposal and cost schedule) (Attach Price or Cost Analysis)
BUILDING/LOCATION: 2400 Lombard Avenue, Everett, WA 98201
This change order increases the total project price by ordering and installing different light poles than what was
originally specified in the solicitation.
The original light poles specified were motion-sensored, with lighting decreasing if no motion was detected. After
the contract award, the noticed that the solicited light poles were not uniform with the installed
light poles in downtown Everett. The newly specified, change order light poles will not have motion sensor controls
and will instead constantly emit light. Having uniform light poles throughout the city will allow city employees to
maintain and repair them with minimal training. Additionally, the city will be able to keep a lower stock of
replacement parts on hand that can be used on these types of light poles.
The project manager coordinated with the awarded contractor to identify and order the correct light poles.
The changes described above result in the following adjustment(s) in price and contract time (in calendar days).
* Total Project Price Prior to this Change,
including tax $377,522.99 Contract Time & Completion Date Prior
to This Change 180
Completion Date
Prior
05/18/25
* Net (Increase) or (Decrease) Resulting from
this Change, including tax +$29,153.30 Net (Increase) or (Decrease) Resulting
from this Change 0
* Total Project Price Including this Change,
including tax $406,676.29 Contract Time & Completion Date
Including This Change 180
Completion Date
After
05/18/25
By signing below, City of Everett and the Contractor agree that the Contract work shall be changed to the extent described in
all such
changes, including all direct and indirect costs and impacts. The City of Everett and the Contractor further agree that
Contract time shall be adjusted to the extent shown above. This Change Order only changes the contract between City of
Everett and the Contractor to the extent explicitly provided in this Change Order.
