ADDITION OF PARTY TO THE AGREEMENT <br /> <br />[INSERT NAME]is hereby added as a party to the Agreement, effective as of the date of <br />this Addendum. All references in the Agreement to the term “Participating Agency” shall <br />henceforth be deemed to include [INSERT NAME]. All terms and conditions of the Agreement <br />applicable to the Participating Agency shall now also apply to [INSERT NAME]. <br /> <br />2.
