CITY OF EVERETT <br /> EVERETT Permit Services <br /> WASHINGTON <br /> EXHIBIT A <br /> ANNUAL BLANKET UTILITY PERMIT ACTIVITIES CATEGORIZED <br /> MINOR ACTIVITIES (covered under this Blanket): These activities are considered normal maintenance activities performed by the utility <br /> company or other activities which would not disrupt the City road or traffic patterns in the City right-of-way.Traffic control must be compliant <br /> with current MUTCD Standards and the City's 700 Series Traffic Control Standard Drawings..Minor activities are limited to work that does not <br /> require traffic control for any lanes of travel, permanent restoration, or new infrastructure. These types of activities require a valid Annual <br /> Blanket Utility Permit, but do not require any additional actions prior to the commencement of work in the public right-of-way. See Section <br /> 3.a.in the Annual Blanket Utility Permit language. <br /> Examples: <br /> • Valve adjustment in right-of-way(parking lane,shoulder,sidewalk,unpaved right-of-way only). <br /> • Valve operation and maintenance. <br /> • Leak surveys(parking lane,shoulder,sidewalk,unpaved right-of-way only). <br /> • Valve box maintenance(parking lane,shoulder,sidewalk,unpaved right-of-way only). <br /> • Testing(parking lane,shoulder,sidewalk,unpaved right-of-way only). <br /> • Cathodic testing(parking lane,shoulder,sidewalk,unpaved right-of-way only). <br /> • Repair or replace service located in gravel shoulder(no hard surface restoration involved). <br /> • Inspection and maintenance of existing facilities(located in parking lane,shoulder,sidewalk,unpaved right-of-way only). <br /> • Aerial service connections or extensions for existing utilities only(where no travel lanes are disrupted/no traffic control is required <br /> for service vehicles). Any new utility installation on a utility pole is considered a Major Activity and requires a Franchise Utility <br /> Permit. <br /> • Utility locates(One-Call/Painting for utility identification only) <br /> • Pothole exploration in soft surface where no City of Everett utilities are present. <br /> MAJOR ACTIVITIES(requires additional permitting):These activities are major utility projects within the right-of-way which involve cutting of <br /> pavement, trench excavation, pavement restoration, installation of new infrastructure, and/or disruption of traffic flow within the right-of- <br /> way.These types of activities require a valid Annual Blanket Utility Permit and additional action. For each 'Major Activity'covered under this <br /> permit, a Franchise Utility Permit application package must be submitted via eTRAKiT to the City's Public Works Department and a permit <br /> must be issued by the City prior to commencing work. See Section 3.b. in the Annual Blanket Utility Permit language for specifics on <br /> submitting via the eTRAKiT portal. <br /> Examples: <br /> • Pothole exploration in hard surface or soft surface where City of Everett utilities exist(Public Works Inspector required to be on- <br /> site for entire duration of potholing for city utilities). <br /> • Open cutting of pavement. <br /> • All main extension. <br /> • Plowing,trenching,or directional boring of utilities or cable in the roadway or gravel shoulder. <br /> • Using a vibratory plow,ditchwitch,or backhoe in the roadway. <br /> • Setting a new utility pole and/or anchor(may require Land Use approval and a valid Franchise Agreement). <br /> • Setting a replacement utility pole and/or anchor(PUD only) of like-kind and within 5ft of existing pole's height. Any replacement <br /> pole of a different material or increased height requires Land Use review and approval through the Planning Division prior to <br /> eligibility for Construction Permit submittal. <br /> • Leak repair in hard surface. <br /> • Main repair in pavement or gravel shoulder. <br /> • Installation of a new service line;or repair to an existing service line with a hard surface cut. <br /> • All new infrastructure installation. <br /> • Replace or install valves in pavement where pavement restoration will take place. <br /> • Directional boring with utility crossing. <br /> • Emergency Gas Leak Repair by PSE/Infrasource is the only permit type eligible to commence work prior to receiving permits, but <br /> must follow-up with a permit application as soon as reasonably possible. <br /> • Installation of antenna equipment on utility poles(may require prior Land Use approval and a valid Franchise Agreement). <br /> • Installation of company equipment on utility poles(may require prior Land Use approval and a valid Franchise Agreement). <br /> • Installation of small-cell technology in Right-of-Way(may require prior Land Use approval and a valid Franchise Agreement). <br /> Page 8 of 8 <br />
