Exhibit A (Scope of Work) <br />SCOPE OF WORK <br />Work to Be Performed - <br />Each City of Everett CHIP housing rehabilitation project, in which the structure was built <br />prior to 1978, must be tested for the presence and condition of Lead Based Paint on <br />every interior and exterior surface. The Service Provider may perform Lead Based Paint <br />risk assessments and clearance inspections, and/or a combinlation of work listed below. <br />Work includes: <br />• NVL Laboratories, Inc., providing an EPA Certified Inspector/Risk Assessor to perform <br />all work <br />• Site testing with the XRF Spectrum Analyzer and two copies of Lead Based Paint <br />Survey test results <br />• If Lead Based Paint is present, a definitive scope of work and options are provided for <br />CHIP Housing Inspector/Advisor and the contractor’s use <br />• Inspection of the project during the course of the work and at the end, performing all <br />required clearance sampling and providing one bound copy and one electronic copy of <br />the report <br />• Performing wipe and soil sampling, including delivery costs and lab analysis <br />• Prepare scope of work for Lead Based Paint bidding purposes, if applicable <br />• Solicit contractors for bidding and conduct project walk-through, if applicable <br />• Receive contractor bids and prepare analysis for Owner of project, if applicable <br />• On site observation of contractor to ensure compliance of regulations <br />• Teach HUD approved lead RRP safety training program (renovation, repair, & <br />painting). <br />• On site air monitoring, if applicable <br />
