City of Everett 00 63 63 - 4 <br />Water Filter Plant Emergency Generator Replacement WO# 3587 <br /> <br /> <br />00 63 63 - 4 CHANGE ORDER – AGREED (Addendum 1) <br /> <br /> <br /> <br />Change Order No. 3 <br />Change Order Effective Date: 1/24/2025 <br /> <br />Exhibit A—Description of Changed Work <br /> <br />Change Order #3 adds two (2) new items to the contract as follows: <br />New Items <br />New Item #109 – Work Change Directives 44-50, 52, 53. Create new item #109 entitled “Work <br />Change Directives 44-50, 52, 53” by adding a lump sum line item in the amount of $160,260.39. <br />New Item #110 – Equitable Adjustment 2. Create new item #110 entitled “Equitable Adjustment <br />2” by adding a lump sum line item in the amount of $39,151.42. <br />Justification <br />New Item #109 – Work Change Directives 44-50, 52, 53. Additional scope of work items described <br />by Work Change Directives 44-50, 52, 53 were required to complete project construction. The <br />combined cost of the Work Change Directives was $160,260.39. The scope of work included in <br />the Work Change Directives includes: <br />• Fiber optic communication system testing. <br />• Motor control center power and control system installation and commissioning. <br />• Generator fuel system improvements. <br />• Installation, coordination, and dismantling of temporary power system. <br />• Generator louver actuator improvements. <br />• Generator control system testing and programming. <br />Further explanation of the new item is shown under Work Change Directives 44-50, 52, and 53 <br />on the attached justification documents. <br />New Item #110 – Equitable Adjustment 2. Continuing delays in project startup and commissioning <br />which are outside of the control of both the City of Everett and the project contractor have <br />prevented the project contractor from completing project construction. As such, City of Everett <br />staff and the project contractor have negotiated an equitable adjustment settlement in the amount <br />of $39,151.42. Further explanation of the new item is shown under Work Change Directives 51, <br />54, 55, and 56 on the attached justification documents. 740 calendar days are added to the <br />contract time as part of Equitable Adjustment 2. The contract time adjustment is a cumulative <br />adjustment covering all project delays resulting from equipment procurement delays, third party <br />startup and commissioning delays, and Work Change Directives 1 – 56 as described in Change <br />Order 1, Change Order 2, and Change Order 3. <br />
