Proving What’s Possible in Healthcare <br />This document contains important terms and conditions which are incorporated by reference into and becomes <br />part of the terms of your contract with us. ©2024, Healthcare Management Administrators, Inc. <br />Care Navigation <br />Product Description Price <br />Care <br />Navigator <br />Plus <br />Our Care Navigator Plus builds upon all the features of our base Care <br />Navigator product and engages members early in the plan year with a health <br />questionnaire to collect good contact information, help identify at-risk <br />members, and pair them with a Care Navigator who may guide them to high- <br />quality, high-value sites of care for surgeries and imaging, in-network primary <br />care, and partner programs. Care Navigator Plus supports the financial health <br />of your organization by optimizing overall spend on high-cost care and <br />improves health outcomes by empowering members at important moments in <br />their health journeys. <br />Clients choose between reducing the member health plan contributions or <br />digital gift cards as an incentive for those members who complete their health <br />questionnaire within the specified timeframe determined by the Plan. <br />The incentive and the health questionnaire are required. <br />We have 3 employer-funded incentive options available. <br />1.Employer Managed Incentive: (We will provide reporting of members <br />qualifying for the incentive via BenefitFocus, employer assumes all liability <br />for the incentive design and execution.) <br />2.$25 digital gift card automatically sent to member-provided email from <br />Tango Platform upon employer funding of Tango account. (We set up <br />automation, client signs 3-way agreement, then client directly funds gifts <br />cards thru Tango.) <br />3.$50 digital gift card automatically sent to member-provided email from <br />Tango Platform upon employer funding of Tango account. (We set up <br />automation, client signs 3-way agreement, then client directly funds gifts <br />cards thru Tango.) <br />Member access to Healthcare Bluebook Quality and Go Green Rewards is <br />included at no added PEPM. <br />•Employer funds Healthcare Bluebook rewards ranging from $25-$100 <br />when a member receives services from a fair price (green) provider. (HCBB <br />mails a reward check directly to members. We bill clients via pass-through <br />admin invoice for paid rewards.) <br />•HCBB Engagement Rewards are not required. They are an available <br />reward upgrade option with no added PEPM. <br />•HCBB Care Connect cannot be sold with Care Nav Plus. <br />The cost of any earned rewards and incentives are not included in the $4 <br />PEPM. <br />$4.00 <br />PEPM <br />Plus the cost <br />of rewards <br />and incentive <br />s <br />Care <br />Navigator <br />Care Navigator is our base service that supports members across their health <br />journey. Our team acts on certain prior authorization requests to steer <br />planned out-of-network care in-network, makes pre-and post-procedure <br />calls, monitors utilization trends to identify opportunities for ER diversion, and <br />educates members on plan benefits to promote utilization and support the <br />health of your population. <br />$2.00 PEPM <br />6
