Proving What’s Possible in Healthcare <br />This document contains important terms and conditions which are incorporated by reference into and becomes <br />part of the terms of your contract with us. ©2024, Healthcare Management Administrators, Inc. <br />Healthcare Bluebook (HCBB)Rewarding Healthcare Consumerism <br />Product Description Price <br />HCBB <br />Quality + <br />Go Green <br />Rewards <br />Healthcare Bluebook offers a premium digital, self-service, cost and <br />quality comparison tool making it easy for members to find the fair <br />price for medical procedures, advanced imaging services, and the <br />nearest and best quality providers for hundreds of inpatient <br />procedures.Bluebook empowers members with greater visibility <br />on cost and quality variance and protects the Plan from overpaying <br />for health services. <br />The ‘Go Green rewards' is the base incentive program that <br />includes 200 eligible procedures and offers reward amounts <br />ranging from $25 -$100 per procedure.Color-coded rankings help <br />members identify the high quality, fair price. <br />This program rewards all members that choose a green (FairPrice) <br />provider. Members receive check via mail. <br />$2.25 PEPM plus the <br />cost of rewards <br />Rewards are billed <br />monthly via the fee <br />request <br />HCBB <br />Quality + <br />Engagement <br />Rewards <br />This product leverages the same HCBB cost and quality <br />comparison tool mentioned above. <br />The 'Engagement Rewards'is an enhanced and tiered incentive <br />program with an expanded selection of over 400 procedures <br />and higher reward amounts ranging from up to $350 for <br />outpatient procedures to a max of $1,000. Color-coded rankings help <br />members identify the high quality, fair price. <br />This program rewards members who shop for a procedure (up to 12 <br />months prior to date of service) and choose a green/yellow <br />cost,green quality provider for a rewardable procedure.Members <br />receive check via mail. <br />$2.25 PEPM plus the <br />cost of rewards <br />Rewards are <br />billed monthly via the <br />fee request <br />Healthcare <br />Bluebook <br />CareConnect <br />(Not available <br />in combination <br />with Care <br />Navigator Plus, <br />not <br />recommended <br />for Care <br />Navigator) <br />Care Connect: concierge service for joints, spine, and women’s <br />surgical procedures health including finding the right provider, <br />scheduling appointment and facilitating medical record transfer. <br />(Only available with Engagement Rewards). Requires client to <br />provide email addresses of all eligible employees.
