1 of 8 <br /> LONGITUDE <br /> ONE TWENTYO <br /> ENGINEERING&DESIGN <br /> Helical/Concentric Pile Re-Inspection Verification Review for Structural Compliance <br /> Date: May 28, 2024 <br /> Project Name: 1612 Rockefeller Ave. - Foundation Stabilization Effort L120 Project#S220725-3XR <br /> Project Location: 1612 Rockefeller Ave., Everett, WA <br /> Client's Name: Alesha Stickles, Project Coordinator—R&R Foundation Specialist, LLC <br /> To Whom It May Concern: <br /> The purpose of this letter is to confirm pile installation and capacity compliance with the structural <br /> requirements specified in the L120 Structural Calculation Package and Structural Site Plan dated <br /> September 22nd, 2022. <br /> During the week of April 261", 2023,fourteen (14) helical and three (3) concentric piles were installed by <br /> R&R Foundation Specialists (R&R)to stabilize areas of the single-family residence located at 1612 <br /> Rockefeller Ave, Everett, WA 98201. Helical Piles were installed with a Digga 10K drive head with a <br /> maximum depth of 25ft, by R&R certified installers in accordance with the cited structural package and <br /> ESR-3750. Concentric Piles were installed utilizing the effective area of the Power Team C25C hydraulic <br /> cylinder used with Stabil-Loc Foundation Piering Systems assembly with a maximum depth of 8ft by R&R <br /> certified installers in accordance with the cited structural package and ESR-4121. <br /> On May 24th, 2024,to align with jurisdiction requirements,the FOR from L120 Engineering and Design <br /> provided an on-site re-inspection where a single installed test pile (Helical Pile#10) was load tested to <br /> be in compliance with the minimum required loading for the typical piles as specified in the structural <br /> calculations. During the re-inspection testing, it was observed that the test pile reached test pressures <br /> equivalent to a load capacity of 29kips before the existing structure showed signs of lifting, at which <br /> point the testing was halted. As can be seen by the initial installation drive logs (attached for reference), <br /> pressures equivalent to a minimum 17.2kips were achieved for the helical pile#10 location during initial <br /> driving of piles. By inspection, it can be seen in both the initial installation results and the re-tested pile <br /> results that the pile capacity of the installed pile exceeds the maximum loading demand of pile#10 and <br /> all typical piles as determined by L120's analysis.Therefore,the installed pile complies with the <br /> structural design/demand. <br /> •6)790 9502 <br />
