<br /> <br />SECTION 4 – PROPOSAL SUBMITTAL REQUIREMENTS <br />4.1 SUBMITTAL REQUIREMENTS <br />Suppliers must provide a proposal which must demonstrate an understanding of the project requirements <br />as stated throughout this Request for Proposal. <br />Proposals in response to this RFP must be submitted in the order specified below. Proposal responses <br />must include: <br />1. Form 4.01 Supplier Commitment and Information (included) <br />2. Form 4.02 Monetary Proposal (included) <br />3. Form 4.03 Questionnaire – Narrative responses to the questions asked. Suppliers should re-type <br />the heading, question identifier and question. Then answer the questions and provide in the same <br />order requested below. Suppliers may emphasize in their narrative any areas of their proposal that <br />they believe exceed our requirements. <br />4.
