CITY OF EVERETT <br /> CONTRACT FOR <br /> BASIC LIFE SUPPORT AMBULANCE SERVICES <br /> This Contract is made effective (the"Effective Date"), and entered into by and between <br /> the City of Everett("Everett" or"City"),a Washington municipal corporation; and Platinum <br /> Nine Holdings, LLC dba NW Ambulance ("Contractor"), a Domestic Limited Liability Company <br /> of the State of Washington,and authorized to do business in the State of Washington. <br /> Contractor Business Platinum Nine Holdings, LLC dba NW <br /> Ambulance <br /> Name of Jeremy Yoder, General Manager <br /> Representative <br /> Contractor Address 706 Cedar Ave, Marysville WA 98270 <br /> Contractor Phone 425-877-3417 <br /> Contractor Fax 360-386-8426 <br /> Contractor E-mail jyoder@nwamb.us <br /> WHEREAS,the purpose of this contract is to provide the City of Everett with Ambulance <br /> Services; and <br /> WHEREAS, Contractor was selected as a result of a Request for Proposal 2015-097; <br /> NOW,THEREFORE, in consideration of the terms, conditions, covenants, and performance of <br /> the scope of work contained herein, as attached and made a part hereof, Everett and Contractor <br /> mutually agree as follows: <br /> Contractor shall provide city-wide ambulance transport services for Basic Life Support (BLS) <br /> patients that have been screened and medically evaluated by the Everett Fire Department(EFD) <br /> and transferred to the Contractor for transport to an appropriate medical facility. The EFD will <br /> remain the first responder for all 9-1-1 calls,will perform triage and interrogation of all patients <br /> and provide Advanced Life Support (ALS)treatment and transport. <br /> 1.0 Definitions: <br /> "At Scene Time"means the point in time when the ambulance unit is physically stopped or <br /> staged at the correct scene. In situations where the unit has been directed to respond to a location <br /> other than the scene, e.g., staging areas for hazardous materials/violent crime incidents, or non- <br /> secured scenes, "at scene time" shall be the point in time the ambulance unit reports it has arrived <br /> at the designated staging location.
