City of Everett Human Needs Grant <br /> January 1, 2016 through December 31, 2016 <br /> Cocoon House <br /> Cocoon House Central: Emergency Teen Shelter <br /> EXHIBIT A: Program Summary <br /> Cocoon House Street Outreach Program connects with homeless and at-risk youth, age 12—24, to help <br /> them get off the streets, locate safe housing, and increase connections to the community. The Street <br /> Outreach Program serves approximately 1,500 youth annually through mobile street-based outreach in all <br /> regions of Snohomish County and through stationary outreach at the agency's Day Center in Everett: U- <br /> Turn. Street Outreach Services are now provided 24 hours a day/7 days a week with the addition of the <br /> National Safe Place initiative. Additionally, the Day Center operates seven days per week providing <br /> meals, showers, laundry facilities, basic needs support, life skills training, connection to community <br /> resources, and intensive case management. <br /> The Cocoon House Street Outreach Program will provide Tangible Basic Needs Assistance for at least <br /> 500 at-risk and homeless youth. The project will provide at least 725 units of Basic Needs Assistance: <br /> showers, laundry, meals, food, clothing, fees for documents, etc. <br />
