<br /> <br />1 <br /> <br />DELEGATE PHYSICIAN AGREEMENT <br />This DELEGATE PHYSICIAN AGREEMENT (the “Agreement”) is entered into by and <br />between the City of Everett (the “City”) and Dr. Ronald Brown (the “Physician”) for services as a <br />Delegate Physician. <br />I. RECITALS <br />WHEREAS, the purpose of this Agreement is to provide delegate physician services to <br />the City of Everett Fire Department for delegate physician services; <br />WHEREAS, The Snohomish County Medical Program Director may delegate to the <br />Physician the duties set forth in WAC 246-976-920(3)(a), 4(a) (c), and (d), 5(a), (e), and (f), 6(a) – <br />(c) and (e), 7(a) – (d) and (f), and 8 (b) – (c) to be performed for those agencies which contract <br />for delegate physician services; and <br />WHEREAS, the City and Physician are desirous of having Physician provide, on a regular <br />and ongoing basis, delegate physician services to the Everett Fire Department. <br />NOW, THEREFORE, in consideration of the mutual covenants and agreements <br />hereinafter set forth and other good and valuable consideration, the receipt and sufficiency of <br />which is hereby acknowledged, the parties agree as follows: <br />II. TERMS <br />1. TERM. This Agreement shall commence on the date of last signature below and remain <br />in effect until terminated as provided herein. <br />2. SERVICES. Physician agrees to provide the services identified in Exhibit A (the <br />“Services”). Physician shall, at all times, observe and comply with generally accepted practices <br />and standards, and comply with all applicable laws and regulations, when providing Services <br />under this Agreement. Physician shall perform the Services in strict conformance with currently <br />approved practices in this field of medicine and in a competent and professional manner <br />consistent with the Washington State Department of Health requirements. <br />3. QUALIFICATIONS. Physician shall be a physician licensed to practice medicine in <br />Washington with knowledge and experience in the administration and management of <br />emergency medical care in the pre-hospital, emergency medical systems environment, and <br />trauma program with preferred BC/BE in emergency medicine. Physician shall meet all criteria <br />of RCW 18.71.205(4) to accomplish the professional objectives of the Secretary of the <br />Washington State Department of Health Services as provided in RCW 18.73.081(5). Physician <br />shall maintain all customary narcotics and controlled substances numbers and licenses. <br />Physician shall be familiar with both state and federal laws and regulations governing <br />Snohomish County Emergency Medical Services (EMS) and trauma services and shall be <br />responsible for compliance with these laws and regulations. <br />4. COMPENSATION. The City agrees to compensate Physician $5800 monthly, in arrears.. <br />The monthly rate shall be increased annually beginning every January by the percentage <br />increase (June to June) in the Consumer Price Index for All Urban Consumers (CPI-U) in the <br />Seattle-Everett Area or successor index, up to a maximum of 3.5% This means, for example,
