Page 14 of 25 <br />SECTION 4 – PROPOSAL SUBMITTAL REQUIREMENTS <br />4.1 SUBMITTAL REQUIREMENTS <br />Suppliers must provide a proposal that must demonstrate an understanding of the project requirements as <br />stated throughout this Request for Proposal. <br />Proposals in response to this RFP must be submitted in the order specified below. Proposals must include <br />the following forms which are included as part of the solicitation: <br />1. Supplier Commitment and Information <br />2. Price Sheet <br />3. Narrative responses to the questions asked. Suppliers should re-type the heading, question <br />identifier, and question. Then, answer the questions and provide in the same order requested below. <br />Suppliers may emphasize in their narrative any areas of their proposal that they believe exceed our <br />requirements. <br />4. Certificate of Non-Debarment/Suspension <br />5. Certification Regarding Lobbying by Contractor <br />6. Disclosure of Lobbying Activities <br />7. Signed SAMHSA Clauses <br />4.2 SUGGESTED RESPONSE FORMAT <br /> Standard 8 1/2" x 11" paper <br /> Single or double sided, numbered pages <br /> Typed with a minimum of 12-point font <br /> Form 4.03 – re-type the question before responding. <br /> <br />EXHIBIT D
