Since the City is using the <br />optional DNS process, this may be the only opportunity to comment on the proposed MDNS. <br />Application Date: April 18, 2016 Complete June 30, 2016 <br />Applicant: Providence Regional Medical Center <br />Michael Gaffney <br />1321 Colby Ave <br />Everett, WA 98201 <br />Project Location: 1815 13th Street <br />Project Description: Construction of a 36,600 s.f. medical clinic with surface and below building parking. <br />Construction of new temporary surface parking for use by the larger hospital campus. <br />Required City Permits: SEPA Determination, Public Works Permit, Building Permit <br />Other Agency Permits: NDPES — Department of Ecology, Air Quality Permit - Puget Sound Clean Air Agency <br />Environmental Documents and Studies: An environmental checklist, proposed MDNS, traffic and parking study and <br />project plans are available for review at the Everett Planning Department at 2930 Wetmore Ave. #8-A, Everett, WA <br />98290. <br />Public Comment Period: Written comments on the application and the proposed MDNS are solicited and must be <br />received at the Everett Planning Department at the address above by August 5, 2016.
