(Lead agencies may modify this form to include additional specific information on project <br /> description.) <br /> Site is a hulk fuel storage facility operated since approximately mid-1980's. Three 30,000-gallon <br /> underground storage tanks (USTs), a fuel dispenser and an overhead loading rack currently <br /> exist on site. One single-story combined office and shop building is also located on Site. This <br /> project includes removing all three USTs, the dispenser and loading rack, all associated buried <br /> piping, and surface concrete. Backfill equal to the tank volumes will be compacted in the pit to <br /> match existing grade, and the Site repaved with asphalt. Minimal contaminated soil is <br /> anticipated with minor excavation of soil beyond removal of the USTs. Entire project is <br /> anticipated to require 8 weeks to complete. <br /> 12. Location of the proposal. Give sufficient information for a person to understand the precise <br /> location of your proposed project, including a street address, if any, and section, township, and <br /> range, if known. If a proposal would occur over a range of area, provide the range or <br /> boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic <br /> map, if reasonably available.
