(8) Authorize the closure of various streets on July 4, 2016 from 6 a.m. to 10:30 <br /> a.m., for 5K, 10K, and One Mile Family Fun Runs, sponsored by the YMCA of <br /> Snohomish County. <br /> (9) Authorize the closure of Alverson Boulevard and Rockefeller Avenue, from 5th <br /> Street to Legion Memorial Park, on July 4, 2016, from 6 a.m. to July 5, 2016 at <br /> 2 a.m., for a Colors of Freedom Festival and Fireworks show. <br /> (10) Authorize the closure of Colby Avenue and Wetmore Avenue, 25th Street to Wall <br /> Street, on July 4, 2016 from 6 a.m. to 3 p.m., for a 4th of July Parade. <br /> PUBLIC HEARING: <br /> (11) CB 1605-25 — 3rd and final Reading —Adopt the Proposed Ordinance authorizing <br /> "Supportive Housing" in various residential and commercial zones, amending <br /> Ordinance No. 1671-89 (EMC Title 19, Zoning) and 2530-01 (EMC Title 15, Local <br /> Project Review Procedures), as amended. <br /> ACTION ITEMS: <br /> (12) Award Request for Proposal 2015-097 for Ambulance Transport Services and <br /> Authorize the Mayor to sign the Contract with Platinum Nine Holdings LLC, doing <br /> business NW Ambulance in the amount of $158,167.04, substantially in the form <br /> provided. <br /> (13) Approve the Application for Public Fireworks Display permits from Wolverine <br /> West, LLC, for the 2016 summer Aquasox games. <br /> (14) Authorize the Mayor to sign the Agreement with Officer Brandon Gill to purchase <br /> retired K-9 Officer Quay in the amount of $10.00. <br /> (15) Authorize the Mayor to sign Amendment No. 2 to the Professional Services <br /> Agreement with ICF Jones & Stokes, Inc. to extend the contract completion date <br /> to June 15, 2018 at no additional cost. <br /> (16) Authorize the Mayor to sign Amendment No. 4 to the Professional Services <br /> Agreement with Carollo Engineers for the Water Pollution Control Facility Phase C <br /> expansion to extend completion of the work to October 31, 2016 at no additional <br /> cost. <br /> (17) Authorize the Mayor to sign the Grant Agreement with Snohomish County Human <br /> Services in the amount of $23,000 to upgrade the existing video monitoring <br /> system at the Carl Gipson Senior Center. <br />
