EXHIBIT A <br />PROFESSIONAL SERVICES AGREEMENT <br />SCOPE OF WORK <br /> <br /> <br /> <br />Exhibit A (Scope of Work) <br />PROJECT DESCRIPTION <br /> <br />Everett Public Main Library Building Envelope Improvements Hazardous Material Survey at 2708 Hoyt <br />Avenue, Everett, WA 98201 <br /> <br />Krazan & Associates, Inc. will provide the City of Everett a limited hazardous material survey as a matter <br />of due diligence prior to renovation activities. The survey for the Everett Main Public Library will be <br />limited to proposed areas of construction indicated on the provided site plan. Current federal, state, and <br />local regulations require that property owners, prior to renovation and/or demolition work on existing <br />properties, conduct surveys to locate and identify hazardous or potentially hazardous materials. The <br />scope of this limited hazmat survey has been client-defined and is intended to provide additional <br />information to the City of Everett as an integral part of their due diligence process. The findings of such <br />surveys are required to be made available to contractors and subcontractors providing services should <br />renovation or demolition be undertaken at the site. Additional codes, standards, and regulations dictate <br />the handling of those hazardous materials identified during the survey. The proper identification and <br />assessment of such materials also facilitate the development of plans and technical specifications that <br />minimize the potential liabilities of owners and contractors. <br /> <br />LIMITED HAZMAT SURVEY <br /> <br />Krazan proposes to conduct the limited hazmat survey to identify suspect asbestos-containing materials <br />(ACM) which may be impacted during the proposed renovations. To comply with the current <br />requirements of federal, state, and local regulatory agencies, Krazan anticipates that the hazmat survey <br />will involve destructive sampling techniques and selective demolition to access hidden or concealed <br />suspect ACM. However, it is understood that the sampling will be limited in scope based on the approval <br />of the property owner and areas defined by the client. The property owner will verify and authorize <br />selective areas for destructive sampling. Arrangements must be made by the property owner or other <br />authorized personnel to provide access to potential sampling areas. If areas of suspect ACM are <br />attainable for sampling, those areas will be included in the report as presumed ACM and will require <br />sampling before any renovations and/or demolition. Repair of selective demolition areas is beyond the <br />scope of these services. Krazan proposes to perform this work on a time and expense basis. <br /> <br />Krazan proposes to perform this work on a time and expense basis. Krazan will collect and submit <br />samples of all suspect ACM encountered at the sites to an accredited and approved laboratory. Upon <br />receipt of laboratory results, Krazan will prepare a written report summarizing findings, conclusions, and <br />recommendations. Krazan will only include areas of the structures that have been approved by the <br />owner of the property and defined by the client. These surveys will be conducted by an Asbestos Hazard <br />Emergency Response Act (AHERA) Building Inspector. <br /> <br />COST AND TIME ESTIMATE <br /> <br />These proposed fee estimates are based on our current understanding of the proposed work being <br />immediate need, and on verbal requests of the proposed scope by the City of Everett to only sample <br />areas allowed by the owner, and the anticipated level of effort that industry-standard experience has
