S5. REPORTING AND RECORDKEEPING REQUIREMENTS <br /> A. High Turbidity Reporting <br /> Anytime sampling performed in accordance with Special Condition S4.0 indicates turbidity has <br /> reached the 250 NTUs or more (or transparency less than or equal to 6 cm), high turbidity <br /> reporting level,the Permittee must notify Ecology within 24 hours of analysis either by calling <br /> the applicable Ecology Region's Environmental Report Tracking System (ERTS) number by <br /> phone or by submitting an electronic ERTS report(through Ecology's Water Quality Permitting <br /> Portal (WQWebPortal)—Permit Submittals when the form is available). Discharge Monitoring Reports (DMRs) <br /> Permittees required to conduct water quality sampling in accordance with Special Conditions <br /> S4.0(Turbidity/Transparency), S4.D (pH), S8 (303[d]/TMDL sampling), and/or G12 (Additional <br /> Sampling) must submit the results to Ecology. <br /> Permittees must submit monitoring data using Ecology's WQWebDMR web application <br /> accessed through Ecology's Water Quality Permitting Portal. <br /> Permittees unable to submit electronically(for example,those who do not have an internet <br /> connection) must contact Ecology to request a waiver and obtain instructions on how to obtain <br /> a paper copy DMR at: <br /> Department of Ecology <br /> Water Quality Program - Construction Stormwater <br /> PO Box 47696 <br /> Olympia, WA 98504-7696 <br /> Permittees who obtain a waiver not to use WQWebDMR must use the forms provided to them <br /> by Ecology; submittals must be mailed to the address above. Permittees must submit DMR <br /> forms to be received by Ecology within 15 days following the end of each month. <br /> If there was no discharge during a given monitoring period, all Permittees must submit a DMR <br /> as required with "no discharge" entered in place of the monitoring results. DMRs are required <br /> for the full duration of permit coverage (from the first full month following the effective date of <br /> permit coverage up until Ecology has approved termination of the coverage). Records Retention <br /> The Permittee must retain records of all monitoring information (site log book, sampling <br /> results, inspection reports/checklists, etc.), Stormwater Pollution Prevention Plan, copy of the <br /> permit coverage letter(including Transfer of Coverage documentation) and any other <br /> documentation of compliance with permit requirements for the entire life of the construction <br /> project and for a minimum of five (5)years following the termination of permit coverage. Such <br /> information must include all calibration and maintenance records, and records of all data used <br /> to complete the application for this permit.This period of retention must be extended during <br /> Construction Stormwater General Permit Page 17 <br />
