The SLA was also mentioned but was not submitted <br />to start back and forth revisions. <br />New Response: <br />The next submittal will need to be a complete package that can <br />be issued for construction to include all plans including civils <br />(Mastec/AT&T plans), illumination plans, pole manufacturer's <br />specs, antenna & equipment, marked up SLA, and any other <br />document that was prepared for construction. This next <br />resubmittal also needs all plans to be coordinated with each other <br />(backgrounds and designs should match and not be <br />contradicting. All plan sets and documents submitted need to be <br />revised to correctly call out the permit address of 4030 Colby <br />Ave. There are currently several inconsistencies throughout the <br />various documents. Revisions are required to get the plans <br />synced with each other. <br />New Comment: <br />The latest submittal turned in to the City was again incomplete. <br />This resubmittal package was still missing the pole specs, site <br />license addendum (SLA) and cost estimate for permit fee <br />calculation. Additionally, the plans are still not coordinated and <br />have several contradictions. <br />Responses such as the one on the right are not acceptable, as <br />the response column is for the entire design team and plans <br />should not be resubmitted until ALL ITEMS have been thoroughly <br />addressed. The applicant/ownership team needs to ensure that <br />all design parties have done their part to update their scope of the <br />plans prior to responses being added and resubmittal. <br />Page 2 of 8 <br />
