How does your company address repairs that cannot be completed on-site at the truck location? <br />Q. What is your published exterior legal “noise level” on the complete truck? <br />3. Communication, Customer Services, and Training <br />A. Describe how your company or project manager will inform the City of Everett of any issues or <br />challenges related to delivering the drainage and sewerline truck. <br />B. Describe your response timeline to a warranty repair call. <br />C. Describe the repair and parts support that your company offers. <br />D. Explain the inspection process before acceptance and access during manufacturing that the City of <br />Everett requires. <br />E. How does your company determine if a repair is under warranty? <br />F. Provide a proposed comprehensive training program to operators, IT staff, and maintenance <br />personnel. Describe the topic provided and its duration. Identify how the training will be <br />conducted, whether online or onsite. <br />G. Describe any operation, maintenance, repair, and training manuals that your company will provide as <br />part of this RFP. <br />4. Risk, Performance, and Quality Assurance <br />A. Submit no more than three (3) completed relevant project experiences within the past five years <br />that demonstrate successful contract performance similar in size and scope to those described in <br />this RFP, including any government experience. Include the following for each reference: <br />a. Company name and full address. <br />b. Point of contact name, title, e-mail address, and phone number. <br />c. Contract title, number, start, and completion dates. <br />d. Contract description and order or service details. <br />B. Have you defaulted on any contracts within the past three years or failed to meet contract terms? <br />If so, describe. <br />C. How long has your camera systems been available commercially? <br />D. How long has your software system been available commercially? <br />E. How often are the camera systems upgraded? <br />F. When are upgrades or patches applied? Include particular days and times that software <br />maintenance is scheduled. <br /> <br />
