EXHIBIT A <br />PROFESSIONAL SERVICES AGREEMENT <br />SCOPE OF WORK <br />SITE AND PROJECT DESCRIPTION <br />Krazan & Associates, Inc will provide the City of Everett a limited hazmat survey on the referenced property which <br />consists of one single-story Recreation Building, No. 229, located within Forest Park. Krazan understands that the <br />City of Everett has requested this survey as a matter of due diligence prior to demolition activities. Current federal, <br />state, and local regulations require that property owners, prior to renovation and/or demolition work on existing <br />properties, conduct surveys to locate and identify hazardous or potentially hazardous materials. The scope of this <br />limited hazmat survey has been client-defined and is intended to provide additional information to the City of <br />Everett as an integral part of their due diligence process. The findings of such surveys are required to be made <br />available to contractors and subcontractors providing services should renovation or demolition be undertaken at <br />the site. Additional codes, standards, and regulations dictate the handling of those hazardous materials identified <br />during the survey. The proper identification and assessment of such materials also facilitate the development of <br />plans and technical specifications that minimize the potential liabilities of owners and contractors. <br />LIMITED HAZMAT SURVEY <br />Krazan will conduct a limited hazmat survey to identify suspect asbestos-containing materials (ACM) and lead <br />based paints (LBP), which may be impacted during any future renovations and/or demolition. To comply with the <br />current requirements of federal, state, and local regulatory agencies, Krazan anticipates that the Hazmat survey <br />will involve destructive sampling techniques and selective demolition to access hidden or concealed suspect <br />ACM/LBP. However, it is understood that the sampling will be limited in scope based on the approval of the <br />property owner, and areas defined by the client. The property owner will verify and authorize selective areas for <br />destructive sampling. If areas of suspect ACM/LBP are unattainable for sampling, those areas will be included in <br />the report as presumed ACM/LBP and will require sampling before any renovations and/or demolition. Repair of <br />selective demolition areas is beyond the scope of these services. <br />Krazan will perform this work on a lump-sum basis. Krazan will collect and submit samples of all suspect ACM/LBP <br />encountered at the site to an accredited and approved laboratory. Upon receipt of laboratory results, Krazan will <br />prepare a written report summarizing findings, conclusions, and recommendations. Krazan will only include areas <br />of the structure that have been approved by the owner of the property and defined by the client. This survey will <br />be conducted by an Asbestos Hazard Emergency Response Act (AHERA) Building Inspector. <br />COST AND TIME ESTIMATE <br />These proposed fee estimates are based on our current understanding of the proposed work being immediate <br />need, and on verbal requests of the proposed scope by the City of Everett to only sample areas allowed by the <br />owner, and the anticipated level of effort that industry-standard experience has shown to be necessary for <br />hazardous materials survey projects that are similar in scope.
