Page 19 of 26 <br />FORM 4.03 QUESTIONNAIRE <br />Suppliers must complete this “Questionnaire,” providing the information in the same order requested <br />below. Suppliers may emphasize any areas of their proposal that exceed our requirements in their <br />narrative. <br />1. Qualifications and Relevant Experience <br />A.Briefly describe your company. Include how long the company has been in business. <br />B.Describe the qualifications of your company, its business experience, and achievements. <br />C.Describe your company’s experience implementing new micro-market vending programs. <br />Specifically address any experience in implementing new programs for public entities. <br />D. If awarded this contract, who are you proposing to be the project or account manager? What <br />are their years of experience, years in the industry, and years with the firm? <br />E. What has your annual staff turnover rate for delivery personnel been for the past three <br />years? <br />F.What characteristics most distinguish your company from your competitors? <br />2. Technical Capability, Approach, and Capacity <br />A. Provide a timeline and implementation plan for this project and include any City of Everett <br />staff time requirements. <br />B.Describe how your company tailors the offerings of each location based on location <br />preferences. Include any tools or techniques used to determine location preferences. <br />C.Describe the vending equipment for each location listed. Specifically, address payment <br />options. Include energy efficiency ratings as applicable. Attach photos if available. <br />1. What connectivity or hook-ups would be required? Include any requested city <br />preparation work. If connectivity is not available, describe alternative options. <br />D. Describe how the services will be approached per location, including: <br />1. Service and restocking schedule, <br />2. Removal of expired products, <br />3. product selection decisions, <br />4. response time for mechanical issues, <br />5. prevention or minimization of out-of-stock and expired products. <br />E.Provide a list of food and beverage options by location that meet the required and preferred <br />offerings listed in Section 2. Include the pricing structure. <br />F.Describe how your company determines its pricing structure for product offerings. Include <br />how product price increases are handled and efforts to stabilize the prices for product <br />offerings. <br />G. Describe your security provisions to minimize shrink and deter or prevent theft and <br />tampering.
