STEP 1. DETERMINE APPLICABILITY <br /> Use the criteria below to determine if this template may be used for your project. If any <br /> criteria below are marked "False," this template may not be used and a report meeting <br /> City standards is required and shall be prepared by a professional Engineer licensed in the <br /> State of Washington.
