Structural Design Associates Engineering Structures to Last SDA#10052.1 October 17, 2024 Keenen Lucas 4020 Hoyt Ave Everett, WA 98201 Subject: Site Visit 4020 Hoyt Ave Everett, WA 98201 SDA Ref. # 10052.1 To whom this may concern: At the request of Keenen Lucas this office visited the subject site to observe the installation of two epoxied holdown anchor bolts. The anchor bolt holes were drilled to the required depth, this depth was verified by this office using a scrap length of copper wire. The holes were brushed and blown with compressed air while a vacuum collected the concrete dust. The outflow pressure of the air compressor was verified as being greater than 80 psi during the blowing. The holes were brushed three to four times between the holes being blown out with air. This process was done a total of three times for each hole. The epoxy being used was verified as the brand and type specified on the plans. The expiration date of the epoxy was listed as June 10, 2026, with a serial number of 1000452984. The epoxy was verified as being adequately mixed prior to placement in the holes. The holes were filled approximately 2/3 full of epoxy. The anchor rods were gently inserted into the holes while being turned slowly until the required depth was reached. Epoxy extruded from around the bolts, onto the sill plate as the anchor was seated. No destructive tests were conducted during this site visit. If during construction it is found that any of the conditions differ from what has been expressed above, notify this office immediately for a revision of design.
