City of Everett 00 63 63 - 4 <br />WFP Air Scour Building Replacement WO# 3813 <br />00 63 63 - 4 CHANGE ORDER – AGREED (Addendum 1) <br />Change Order No.____1____________________ <br />Change Order Effective Date:__9/xx/25_________ <br />Exhibit A—Description of Changed Work <br />Change Order #1 adds one (1) new item to the contract and modifies one (1) bid item as follows: <br />New Item <br />New Item #18 – Air Scour Revisions. Create new item #18 entitled Air Scour Revisions by adding <br />a lump sum line item in the amount of $17,216.08. New item #18 Air Scour Revisions provides <br />payment for WCD-007 Existing Blower Repairs in the amount of $17,045.43 and WCD-008 VFD <br />Cables in the amount of $170.65. Item #18 results in a $17,216.08 contract price increase. <br />Modified Item <br />Bid Item #12 – Crushed Surfacing Top and Base Course. Modify Bid Item #12 Crushed Surfacing <br />Top and Base Course by adding 0.41 tons of crushed surfacing top and base course to Bid Item <br />#12. This results in a $53.30 contract price increase. <br />Justification <br />Item #18 – Lump Sum. <br />WCD-007 – Existing blower is 32-years old. During startup of the blower, there were abnormal <br />bearing noises. No records can be found that document that the bearings have been replaced. <br />New bearing housings are needed to install the city provided vibration and RTD sensors. <br />Replacement of the bearings and bearing housings was recommended by the commissioning <br />technician. <br />WCD-008 – The contact plans and specifications did not require VFD cables. VFD cables <br />protect motors from high frequence voltage spikes and electromagnetic interference caused by <br />the VFDs that control the motors. VFD cables will extend the life of blower motors. <br />Bid Item #12 – Quantities provided in the bid tabulations are the engineer’s estimated quantities. <br />It is common for the actual quantity used to be different than the estimated quantity.
