Functionality <br />The ACCESS Project is a referral or navigation hub to get <br />youth to the most appropriate community resource for the <br />help and support they need. This is accomplished using three <br />(3) primary functions. <br />1)Referrals made by concerned party or self-referrals from <br />the individual <br />2)Intake Process – Conduct comprehensive assessment <br />interviews to determine the root causes of the mental <br />health challenges and identify the most suitable <br />community partner for helping the youth, which may <br />include family members. <br />3)Schedule the appointment with a community <br />organization or professional counseling service provider <br />and personally accompany the youth to their initial <br />introduction.
