City of Everett 00 41 13 - 5 <br />Port Gardner Storage Facility - Facility Construction Package WO# UP-3525 <br /> <br /> <br /> <br /> <br />00 41 13 - 5 BID FORM <br />ALTERNATE BID ITEM SCHEDULE <br />The following alternate bid items are an integral part of this proposal, and to be responsive, the <br />bidder shall quote for the Base Bid, and also for the following alternate bid items: <br /> <br />ITEM DESCRIPTION UNITS ESTIMATED <br />QUANTITY <br />UNIT BID <br />PRICE <br />CONTRACT <br />PRICE <br />A1. Area 30 Guardrail <br />Replacement <br />Lump Sum <br /> 1 N/A $_______.___ <br />9.9% WA STATE SALES TAX FOR ITEM A1 $ <br />ADDITIVE BID $ <br /> <br />The amount bid for Alternative A1 shall be the differential price between the price <br />included in the base bid Item 17 (“Area 30 Guardrail Repairs”) and the price to provide <br />Alternative A1. <br /> <br />At the City’s sole discretion, the Project awarded will be either: <br />1. The Base Bid only, in which case the Project will not include Alternative A1, and the <br />Contract Sum and lowest bidder will be determined by the lowest TOTAL BASE BID; or <br />2. The Base Bid plus Alternate A1, in which case the Project will include Alternative A1, <br />and the Contract Sum and lowest bidder will be determined by the lowest TOTAL BASE <br />BID + TOTAL A1 BID. <br /> <br />The Bid Security is based on the TOTAL BASE BID. <br />1.3 ACCEPTANCE <br />A. This offer shall be open to acceptance and is irrevocable for 45 days from the Bid <br />closing date. <br />B. If this Bid is accepted by the Owner within the time period stated above, we will: <br />1. Furnish the required 100% payment and 100% performance bonds in the <br />form described in Contract Documents within 14 calendar days of receipt of <br />Notice of Award. <br />2. Submit to the City in pdf format the certificate of Insurance and additional <br />insured endorsements, per SECTION 007200 of the Contract Documents, <br />within 14 calendar days of receipt of Notice of Award <br />3. Execute the Contract within 3 business after receipt from the City’s <br />AdobeSign system. <br />4. Commence Work within seven calendar days after receipt of Notice to <br />Proceed.
