RRFB PEDESTRIAN SAFETY October 31, 2025 <br />STATE FUND # HLP-PB25(004) <br />WO 3807 <br />Page 31 of 82 <br />1 MSVWBE Reporting <br />2 An end of project Report of Amounts Paid to MSVWBEs shall be submitted to the <br />3 Engineer after Physical Completion of the Contract. The end of project report is due <br />4 20 calendar days after the physical completion of the project has been issued. <br />5 <br />6 The end of project report shall include payments to all eligible businesses regardless <br />7 of their listing on the MSVWBE Inclusion Plan. If the Contractor is a MSVWBE the <br />8 amounts paid by the Contracting Agency for Work performed by the Contractor shall <br />9 also be reported. <br />10 <br />11 MSVWBE Payment <br />12 All costs for implementation of the requirements for Voluntary MSVWBE Participation <br />13 shall be included in the associated items of Contract Work. <br />14 <br />15 1-07.11(2).RTF <br />16 1-07.11(2) Contractual Requirements <br />17 (November 25, 2024 APWA GSP) <br />18 Delete item 11 of the first paragraph of Section 1-07.11(2). <br />19 <br />20 1-07.17.GR1 <br />21 Utilities and Similar Facilities <br />22 <br />23 1-07.17.INST1.GR1 <br />24 Section 1-07.17 is supplemented with the following: <br />25 <br />26 COE 1-07.18.rtf <br />27 1-07.18 Public Liability and Property Damage Insurance <br />28 <br />29 Delete this section in its entirety, and replace it with the following: <br />30 <br />31 1-07.18 Insurance <br />32 (January 4, 2024 APWA GSP) <br />33 <br />34 1-07.18(1) General Requirements <br />35 A. The Contractor shall procure and maintain the insurance described in all subsections of <br />36 section 1-07.18 of these Special Provisions, from insurers with a current A. M. Best rating <br />37 of not less than A-: VII and licensed to do business in the State of Washington. The <br />38 Contracting Agency reserves the right to approve or reject the insurance provided, based <br />39 on the insurers financial condition. <br />40 <br />41 B. The Contractor shall keep this insurance in force without interruption from the <br />42 commencement of the Contractors Work through the term of the Contract and for thirty <br />43 (30) days after the Physical Completion date, unless otherwise indicated below. <br />44 <br />45 C. If any insurance policy is written on a claims-made form, its retroactive date, and that of <br />46 all subsequent renewals, shall be no later than the effective date of this Contract. The <br />47 policy shall state that coverage is claims made and state the retroactive date. Claims- <br />48 made form coverage shall be maintained by the Contractor for a minimum of 36 months <br />49 following the Completion Date or earlier termination of this Contract, and the Contractor <br />50 shall annually provide the Contracting Agency with proof of renewal. If renewal of the <br />51 claims made form of coverage becomes unavailable, or economically prohibitive, the <br />52 Contractor shall purchase an extended reporting period (tail) or execute another form of
