<br /> <br />CHANGE ORDER/AMENDMENT <br /># <br /> <br />Contract (Procurement) Number: Contract Title: <br />RFQ #2024-035: Overhead Door and Automatic Gate Inspection and Maintenance <br />Description of Change: <br />1. As allowed in the Contract Terms, the City extends the contract for one year. This shall be the <br />3rd contractual year (2nd option period), in which the term of the renewal period will be from <br />4/01/2026 to 3/31/2027. <br />2. As allowed per Section 2.14, Pricing Adjustments, of RFQ #2024-035, pricing shall increase as <br />shown in the attached Appendix A. <br />3. Except as otherwise specified in this Contract Amendment, the terms and conditions of <br />Request for Quotation 2024-035 Overhead Door and Automatic Gate Inspection and <br />Maintenance shall remain in full force and effect. <br />By signing below, City of Everett and the Contractor agree that the Contract shall be changed to the extent described in the <br />above “Description of Change.” This Change Order/Amendment only changes the contract between City of Everett and the <br />Contractor to the extent explicitly provided in this Change Order/Amendment.
