Consent & Release Thank you for helping us share the stories behind clean energy and community renewal. We request your permission to use any information, photographs, video, audio recordings, and related materials that feature you, your business, or your likeness (collectively, “Materials”) in connection with our Fusion short documentary project produced by Future in Bloom. By signing below, you in your capacity as Mayor of the City of Everett (City) grant Future in Bloom permission to take photographs, video, and/or audio recordings of you and to use those Materials, along with your name, voice, quotes, and biographical information, in the ways described below: Terms of Consent 1.Editing and Adaptation The City and I acknowledge and agree that Future In Bloom may edit, adapt, combine, or otherwise modify the Materials for storytelling purposes, provided that such use will not be defamatory to me or misrepresentative of my participation. 2.No Compensation The City and I understand that the City and I will not receive financial compensation for participation or for the use of the Materials, and that the rights granted here are freely given for this documentary and related media. 3.Release of Claims The City and I release Future In Bloom, its partners, agents, and assignees from any claims or liability arising out of the use of the Materials as described in this release. 4.Grant of Rights The City and I, the undersigned, grant Future In Bloom and its representatives the perpetual, irrevocable right and license to record, reproduce, adapt, and distribute the Materials in any format or medium now known or later developed. This includes use in films, promotional content, educational materials, and public distribution worldwide. 5.Authority to Sign I affirm that I am of legal age (18+), have the right to sign this release, and understand its contents and effect.
