PAGE 1 AFFILIATION AGREEMENT Between City of Everett and CLEVELAND STATE UNIVERSITY - COLLEGE OF HEALTH THIS AGREEMENT is entered into this June 9th 2026, by and between City of Everett (hereinafter the “Organization”), and CLEVELAND STATE UNIVERSITY, a state- supported university and instrumentality of the State of Ohio established pursuant to Ohio Revised Code Section 3344, acting by and through its Board of Trustees, and located at 2121 Euclid Ave., Cleveland, Ohio 44115 (hereinafter the “University”), by and through their duly authorized administrators. RECITALS WHEREAS, the University’s School of Social Work desires to provide field experience for students enrolled in its programs (the “Students”); and WHEREAS, the Organization recognizes the need for and desires to assist with this field experience and is willing to make available its facility and resources to the University for such purposes; NOW, THEREFORE, in consideration of the mutual promises and covenants set forth herein, the receipt and sufficiency of which is hereby acknowledged, the University and the Organization (both or either of which may hereinafter be referred to respectively as the “Parties” or a “Party”) agree as follows: I. TERM AND TERMINATION This Agreement is valid for a term of five (5) years, commencing on the Date of this Agreement. Thereafter, the Parties may agree to initiate a new agreement term. Notwithstanding the foregoing, this Agreement may be terminated by either Party at any time upon thirty (30) days prior written notice to the other Party. In the event of any termination pursuant to this section, the Parties will use their best efforts to ensure that any Student then participating in field experience is able to complete his/her field experience. II. UNIVERSITY RESPONSIBILITIES 1. The University, at all times, retains sole responsibility for the educational standards of its programs as established by the School of Social Work, the Council of Social Work Education, and the State of Ohio for Social Work licensure. 2. The University shall be responsible for the approval/assignment of Students to the Organization. The University agrees to assign only those Students to the Organization who have completed the required course of study as determined by the University. 3. The University shall designate a Faculty Liaison who will serve as the University’s liaison with the Organization for all communications concerning all aspects of the field experience, including but not limited to planning, placement and scheduling.
