PAGE 1 <br />AFFILIATION AGREEMENT <br />Between <br />City of Everett <br />and <br />CLEVELAND STATE UNIVERSITY - COLLEGE OF HEALTH <br />THIS AGREEMENT is entered into this June 9th 2026, by and between City of Everett <br />(hereinafter the “Organization”), and CLEVELAND STATE UNIVERSITY, a state- supported <br />university and instrumentality of the State of Ohio established pursuant to Ohio Revised Code <br />Section 3344, acting by and through its Board of Trustees, and located at 2121 Euclid Ave., <br />Cleveland, Ohio 44115 (hereinafter the “University”), by and through their duly authorized <br />administrators. <br /> <br />RECITALS <br />WHEREAS, the University’s School of Social Work desires to provide field experience for <br />students enrolled in its programs (the “Students”); and <br />WHEREAS, the Organization recognizes the need for and desires to assist with this field <br />experience and is willing to make available its facility and resources to the University for such <br />purposes; <br />NOW, THEREFORE, in consideration of the mutual promises and covenants set forth herein, the <br />receipt and sufficiency of which is hereby acknowledged, the University and the Organization <br />(both or either of which may hereinafter be referred to respectively as the “Parties” or a “Party”) <br />agree as follows: <br /> <br />I. TERM AND TERMINATION <br /> <br />This Agreement is valid for a term of five (5) years, commencing on the Date of this Agreement. <br />Thereafter, the Parties may agree to initiate a new agreement term. Notwithstanding the <br />foregoing, this Agreement may be terminated by either Party at any time upon thirty (30) days <br />prior written notice to the other Party. In the event of any termination pursuant to this section, <br />the Parties will use their best efforts to ensure that any Student then participating in field <br />experience is able to complete his/her field experience. <br />II. UNIVERSITY RESPONSIBILITIES <br />1. The University, at all times, retains sole responsibility for the educational standards of its <br />programs as established by the School of Social Work, the Council of Social Work Education, <br />and the State of Ohio for Social Work licensure. <br />2. The University shall be responsible for the approval/assignment of Students to the <br />Organization. The University agrees to assign only those Students to the Organization who have <br />completed the required course of study as determined by the University. <br /> <br />3. The University shall designate a Faculty Liaison who will serve as the University’s liaison <br />with the Organization for all communications concerning all aspects of the field experience, <br />including but not limited to planning, placement and scheduling.
