I <br /> INSTRUCTIONS TO BIDDERS <br /> I <br /> 1.0 Design Engineer <br /> Questions and inquiries about these Specifications and Contract Plans should be directed to the <br /> attention of GraceKane,'`'(425) 257-8855 or gkene@everettwa gov <br /> 2.0 Bidder's Check List <br /> The bidder's attention is directed to the following forms which must be executed in full as required <br /> and submitted with the bid: <br /> 1) Proposal: The lump sum and unit price items must be shown in the space provided. Show <br /> unit prices in figures. <br /> 2) Proposal Signature Sheet: To be filled in and signed by the Bidder. <br /> 3) Local Agency Disadvantaged Business Enterprise Utilization Certification <br /> (DOT Form 272-056A): To be filled in and signed by the bidder (if DBE goal is assigned.) <br /> 4) Local Agency Disadvantaged Business Enterprise (DBE) Written Confirmation <br /> Document (DOT Form 422-031A): To be filled in and signed by DBEs (if DBE goal is <br /> assigned.) <br /> 5) Non-Collusion Affidavit: To be submitted with the bid. <br /> 6) Bid Bond: This form is to be executed by the Bidder and the surety company unless bid is <br /> accompanied by a certified check. The amount of this bond shall be not less than five <br /> percent (5%) of the total amount bid and may be shown in dollars or on a percentage basis. <br /> Cash will not be accepted. <br /> 7) Proposal for Incorporating Recycled Materials into the Project: To be filled in and <br /> submitted with Bid. <br /> 8) Delay Start: This project has a delayed start, please refer to Section 1-08.4 of these Special <br /> Provisions. <br /> Failure to complete the aforementioned forms and to submit said forms with the bid may be due <br /> cause for rejection of bid. <br /> 3.0 Contract Forms <br /> The following forms are to be executed after the award of Contract: <br /> 1. Contract: This Contract to be executed by the successful bidder in duplicate within twenty <br /> (20) calendar days after the award date. <br /> 2. Performance Bond (DOT Form 272-002A): This form is to be executed by the successful <br /> bidder and his surety company in triplicate.
