CITY OF EVERETT SPECIAL PROVISIONS <br /> Stormwater General Permit. File Notice of Intent with Ecology and <br /> develop a Construction Stormwater Pollution Prevention Plan (SWPPP) <br /> prior to beginning construction. Criteria and requirements for developing <br /> a SWPPP can be found in Volume II of the City of Everett's 2010 <br /> Stormwater Management Manual. The Plan can be found on the City's <br /> web site using the following <br /> link:http://www.ci.everett.wa.us/default.aspx?ID=1728 <br /> Obtain a NPDES CONSTRUCTION STORMWATER GENERAL PERMIT from <br /> Ecology since the Project will disturb more than one acre of surface area. I <br /> DOE website: <br /> http://www.ecy.wa.gov/programs/wq/stormwater/construction/index.html <br /> Contact Ecology to determine if other requirements apply to the Project. ! <br /> NW Region DOE phone: 425-649-7000 <br /> Include all cost incurred for obtaining the CONSTRUCTION STORMWATER <br /> GENERAL PERMIT in appropriate bid item or items as no direct <br /> compensation will be made. <br /> 1-07.9 Wages <br /> 1-07.9(1) General <br /> Delete the first paragraph of 1-07.9(1) and substitute the following: <br /> This Contract is subject to the minimum wage requirements of RCW 39.12 and to <br /> RCW 48.28, as amended or supplemented. Workers shall receive no less than <br /> the prevailing rate of wage. Bidder shall use the Washington State Prevailing <br /> Wage Rates for Snohomish County, effective at the time of bid opening. Bidder is <br /> solely responsible to use the schedule in effect at the Bid Opening Date, <br /> determine the appropriate labor classification(s), and use the appropriate and <br /> correct prevailing wage and benefit rate(s). The hourly minimum rates for wages <br /> and fringe benefits can be obtained at the following URL: <br /> http://www.Ini.wa.gov/TradesLicensing/PrevWage/WageRates/default.asp <br /> Printed copies of the current prevailing wage rates are available for viewing at City <br /> of Everett Public Works, 3200 Cedar St, Everett, WA and the City will mail a hard <br /> copy of the prevailing wage rates upon written request received within 7 days of <br /> the Bid Opening Date. <br /> Delete the fifth paragraph of 1-07.9(1) and substitute the following: <br /> If employing labor in a class not listed in the L & I prevailing wage rate schedule, <br /> the correct wage and benefits rate for that class and locality from the Industrial I <br /> Statistician, Washington State Department of Labor and Industries (State L&I), <br /> and provide a copy of those determinations to the Project Engineer. <br /> 1-07.9(5) Required Documents , <br /> Supplement 1-07.9(5) by adding the following: <br /> The Owner will require payroll reports for the Contractor and every Subcontractor 111be submitted weekly to the Construction Division, Public Works Annex, 3101 <br /> Cedar Street, Everett, Washington 98201. The payroll reports shall contain the <br /> following information: <br /> 1. Name and residence address of each worker. <br /> 2.
