PUSL/C WORKS <br /> March 8, 2012 <br /> GREEN TREE PLAZA 06A LLC <br /> 12411 VENTURA BLVD <br /> STUDIO CITY CA 91604 <br /> Subject: Additional Backflow Prevention Requirements for City of Everett site address: <br /> 305 SE EVERETT MALL WAY <br /> Dear Property Owner: <br /> The protection of the City of Everett water system from potential private customer hazards is a <br /> Washington State Department of Health (DOH) requirement and you are being notified by the City of <br /> Everett, the water purveyor for the building address listed in the subject line above, that construction <br /> upgrades are required for your property when there are modifications required to the sprinkler system, <br /> based on future construction or change of use. It is important to us and to our customers that the public <br /> water system delivers high quality potable water in all cases. <br /> City of Everett records indicate that this building is served by a 8-inch single check-valve, located <br /> CASCADE MUSIC - 15' E. OF CENTER OF E. SIDE OF BUILDING. A double check detector is required by <br /> the State of Washington for commercial fire services. To comply with state requirements, the city <br /> requires the upgrade of the single check-valve to a double check detector assembly when the commercial <br /> building sprinkler system is modified through the city permit process. Please be advised that the city <br /> requires this upgrade as a part of building or sprinkler permit application for the portion of the building <br /> served by the current single check valve. <br /> As the owner of this assembly, you are required to maintain it and improve it as necessary.
