EXHIBIT A <br /> Work Order Form <br /> GEOTECHNICAL SERVICES PROJECT SCOPE AND ESTIMATE <br /> Project Title: Phase I ESA—Smith Street Sawmill W.O. # 1 <br /> • <br /> 1. Project Scope: <br /> CONSULTANT will perform a Phase I environmental site assessment (ESA) for the approximate 4.7-acre <br /> Smith Street Sawmill property located at 3600 Smith Avenue (Snohomish County Tax Parcel Numbers <br /> 00-5760-019-000-05, 00-5760-019-000-06, 00-5760-043-000-02, 00-5760-012-000-03, <br /> 29-0529-003-008-00, 29-0529-002-012-00, 29-0529-003-030-00 and 29-0529-003-010-00) in <br /> Everett, Washington. The Phase I ESA is requested to assist City staff in evaluating potential <br /> environmental liabilities associated with ownership and potential redevelopment of the subject property. <br /> The purpose of this Phase I ESA is to identify Recognized Environmental Conditions (RECs) in connection <br /> with the subject property. The scope of services is in general accordance with ASTM International <br /> Standard E 1527-13 for Phase I ESAs and the U.S. Environmental Protection Agency's (EPA's) <br /> Federal Regulation 40 CFR Part 312 "Standards and Practices for All Appropriate Inquiries (AAI)." <br /> The services described below will be completed by,or under the direction of,an environmental professional <br /> as described in 40 CFR Part 312.CONSULTANT will complete the following tasks: <br /> 1. Review readily available geotechnical reports, environmental reports and/or other relevant <br /> documents pertaining to environmental conditions at the subject property. <br /> 2. Review the results of a federal,state, local and tribal environmental database search provided by <br /> an outside environmental data service for listings of properties with known or suspected <br /> environmental concerns on or near the subject property within the search distances specified by <br /> ASTM. Our database and file review search will include a check for and review of publications or <br /> reports on EPA and Washington State Department of Ecology (Ecology) websites concerning <br /> areawide soil and groundwater contamination on or adjacent to the subject property. <br /> 3. Review regulatory agency files regarding listed properties of potential environmental concern <br /> relative to the subject property. <br /> 4. Identify a key site manager with specific knowledge of past and present property use and request <br /> that the key site manager meet a GeoEngineers' representative on-site for an interview during the <br /> visual site reconnaissance and/or an interview by telephone if he or she is not available during the <br /> site reconnaissance. Identify and interview others familiar with the use and history of the subject <br /> property, as available and appropriate, including representatives of current occupants that likely <br /> use,store,treat, handle or dispose of hazardous substances now or in the past. <br /> 5. Interview current owners or occupants of neighboring properties only as necessary to gather <br /> information or fill site use data gaps regarding the subject property or if the subject property is <br /> abandoned and no owner or occupant interviews can be conducted. <br /> 6. Interview past owners and occupants of the subject property as necessary to gather information or <br /> fill property use data gaps regarding property use history. <br /> 7. Interview a representative of the local fire department, health department, and/or Ecology as <br /> necessary to gather information or fill data gaps regarding the history of the subject property and <br /> surrounding properties relative to the likely presence of hazardous substances. <br /> GeoEngineers,Inc. <br /> File No.0661-108-00 <br />
