i <br /> POLICE HQ- MASONRY CLEANING AND REPAIR APRIL 15, 2016 <br /> $ 1,000,000 Disease-Policy Limit <br /> $ 1,000,000 Disease-Each Employee <br /> A stop gap policy limit of$500,000 will be acceptable if, and only if,the Contractor <br /> Excess or Umbrella Liability policy required by 15.3.14.6 Excess or Umbrella Liability <br /> ' provides coverage over the stop gap policy. <br /> 15.3.14.5 Automobile <br /> Commercial Automobile Liability: as specified by Insurance Services Office, form number CA 0001, <br /> Symbol 1 (any auto), with an MCS 90 endorsement and a CA 9948 endorsement attached if"pollutants" <br /> as defined in exclusion 11 of the commercial auto policy are to be transported. Such policy(ies)must <br /> provide coverage with a combined single limit of not less than$1,000,000 for each accident. <br /> ' 15.3.14.6 Excess or Umbrella Liability <br /> The limits stated in this section 15.3.14 may be satisfied by a combination of commercial general liability <br /> and,if necessary,commercial umbrella/excess policies. <br /> ' 15.3.14.7 Pollution Liability <br /> A policy providing coverage for claims involving remediation,disposal,or other handling of pollutants <br /> arising out of Contractor's operations for others; contractors site(owned); arising from the transportation <br /> of hazardous materials;or involving remediation,abatement,repair,maintenance or other work with lead- <br /> based paint or materials containing asbestos. <br /> ' Such Pollution Liability policy shall provide at least$2,000,000 per occurrence coverage for Bodily <br /> Injury and Property Damage. <br /> 15.3.14.8 Worker's Compensation <br /> A policy of Worker's Compensation, as required by the Industrial Insurance Laws of the State of <br /> Washington. As respects Workers' Compensation insurance in the state of Washington, Contractor shall <br /> secure its liability for industrial injury to its employees in accordance with the provisions of RCW Title <br /> 51. If Contractor is qualified as a self-insurer in accordance with RCW 51.14, Contractor shall so certify <br /> by letter signed by a corporate officer indicating that it is a qualified self insured, and setting forth the <br /> limits of any policy of excess insurance covering its employees. <br /> 15.3.15 Self-Insurance <br /> At its sole option and in its sole discretion,Owner may accept Contractor's self-insurance for a liability <br /> coverage in lieu of insurance from an insurer. Contractor must provide a letter from its Corporate Risk <br /> Manager,or appropriate Finance Officer representing and warranting the following minimum <br /> information: whether the self-insurance program is actuarially funded;the fund limits; any excess <br /> declaration pages to meet the Contract requirements;a description of how Contractor would protect and <br /> ' defend the Owner as an Additional Insured in their Self-Insured layer;and claims-handling directions in <br /> the event of a claim. Any amounts due to, sought by,or paid to third party claimants shall be the sole <br /> responsibility of the Contractor, irrespective of whether such amount falls wholly within the level or <br /> amount of the Contractor's self-insured retention. <br /> 16. LAWS,REGULATIONS AND PERMITS <br /> 16.1 GENERAL <br /> The Contractor shall give the Notices required by law and comply with all laws,ordinances,rules and <br /> ' regulations pertaining to the conduct of the Work. The Contractor shall be liable for violations of same in <br /> connection with Work provided by the Contractor; and Contractor shall cooperate with all governmental <br /> entities regarding inspection of the Work and compliance with such requirements.
