On the plan review questionnaire (#16), you have indicated that your 3-compartment sink does not <br />have integral drainboards on each side. Submit the make and model of the 3-compartment sink you <br />will be using that meets this requirement. Be aware that the drainboards must be of equal size to <br />the dimensions of one of the basins. <br />3. On the plan review questionnaire (#38), you have indicated that food will be stored or prepared at <br />arcther lccation, but ha�,e net listed the name and address cf said !ecation. Submit this information. <br />4. On the plan review questionnaire (#40), you have indicated that food will be cooked or prepared <br />outside of the establishment. Be sure this outdoor cooking or prepping area is indicated on your <br />floor plan. <br />Please note that prior to opening of the new facility, after the Health District plan review process is <br />completed and construction is finished, the Health District permit application process must be <br />completed and a preoperational inspection must be conducied. <br />Piease contact me if you have any questions.
