Proposal: The lump sum and unit price items must be shown in the space provided. Show <br /> unit prices in figures. <br /> 2. Proposal Signature Sheet: To be filled in and signed by the Bidder. <br /> 3. Subcontractor List: To be filled in by the Bidder. <br /> 4. Local Agency Disadvantaged Business Enterprise Utilization Certification <br /> (DOT Form 272-056A): To be filled in and signed by the bidder(if DBE goal is assigned.) <br /> 5. Local Agency Disadvantaged Business Enterprise (DBE)Written Confirmation <br /> Document (DOT Form 422-031A): To be filled in and signed by DBEs (if DBE goal is <br /> assigned.) <br /> 6. Non-Collusion Affidavit: To be signed and submitted with the bid. <br /> 7. Bid Bond: This form is to be executed by the Bidder and the surety company unless bid is <br /> accompanied by a certified check. The amount of this bond shall be not less than five percent <br /> (5%) of the total amount bid and may be shown in dollars or on a percentage basis. Cash will <br /> not be accepted. <br /> 1 Failure to complete the aforementioned forms and to submit said forms with the bid may be due <br /> cause for rejection of bid. <br /> 3.0 Contract Forms <br /> The following forms are to be executed after the award of Contract: <br /> 1. Contract: This Contract to be executed by the successful bidder in duplicate within twenty <br /> (20) calendar days after the award date. <br /> 2. Performance Bond (DOT Form 272-002A): This form is to be executed by the successful <br /> bidder and his surety company in triplicate. The amount of this bond shall be one hundred <br /> percent(100%) of the amount of the bid and shall be submitted with the contract. <br /> 3. Payment Bond (DOT Form 272-003A): This form is to be executed by the successful bidder <br /> and his surety company in triplicate.
