D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Housing support for <br /> persons with <br /> developmental <br /> disabilities and their <br /> families in obtaining 20 200 <br /> and retaining safe and <br /> affordable housing <br /> including collaborative <br /> efforts with other <br /> systems and agencies. <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 50 35 <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Staffing <br /> [Add Provide a list of staff and time commitments to be allocated to the activity specified in l.A. above] <br /> A unit of service is defined as: Direct service in assisting with applications applicable to housing for <br /> individuals with developmental disabilities and their families. These could include SSI/SSDI <br /> paperwork, rep payee services, Section 8, rental applications, DDA, CSO, guardianship paperwork. <br /> Assistance given to individuals with developmental disabilities and their families in locating safe and <br /> appropriate housing. Providing workshops, support groups and speaker nights addressing the <br /> various housing resources and proper future planning for parents. The Independent Living <br /> Coordinator is Lynne White who is .75 FTE, .5 office manager/bookkeeping. The Program Director <br /> provides guidance and supervision, all is supervised and monitored by the Executive Director. <br /> A-5 <br />
