D. Levels of Accomplishment- Goals and Performance Measures In addition to the normal administrative services required as part of this Agreement, the Subgrantee agrees to provide the following levels of program services: Activity Units of Service* per Month Total Units/Year Number of Individuals who have been provided 1.88 individuals or .375 22.5 people/4 .5 emergency households households needs/resources Total People or Households to be served Total unduplicated Everett Residents or Households to be served 22.5 people/ 4 .5 Households 22 .5 people/4 .5 Households *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, persons or households assisted, or meals served, here. If this is a construction project, please explain here how the improvements will be used and how they benefit the community, what the target population is for the project, etc. The Subgrantee agrees to track outcome performance measures as required by the U.S. Department of Housing and Urban Development and the City. The Subgrantee will report the outcomes to the City on at least a quarterly basis. Information and forms will be provided to the Subgrantee by the City for reporting this information. E. The term of this Agreement and the provisions herein shall be extended to cover any additional time period during which the Subgrantee remains in control of CDBG funds or other assets, including program income.
